Position Overview

Superb Hospice seeks a dedicated and compassionate Hospice LVN to join our interdisciplinary team of healthcare professionals. In this full-time role, you will deliver hands-on, end-of-life care while supporting patients, family members, and other community caregivers. This position calls for a caring and skilled individual passionate about making a difference in the lives of those facing the challenges of a serious illness.

Key Responsibilities

  • Provide direct patient care to hospice patients, ensuring comfort and quality of life
  • Monitor patient vital signs and report any changes in patient condition to the supervising nurse or physician
  • Educate patients, family members, and caregivers on the disease process, symptom management, and end-of-life care
  • Develop and maintain therapeutic relationships with patients and their families
  • Collaborate with the interdisciplinary team to address patient care needs and coordinate the plan of care
  • Document patient care and assessments in compliance with hospice policies and procedures
  • Participate in ongoing education and professional development as necessary

Qualifications and Skills

  • Must have a valid Licensed Vocational/Practical Nursing license within California
  • Minimum of two years of experience as an LVN; hospice experience is highly preferred
  • Excellent communication and interpersonal skills, with the ability to express empathy and compassion
  • Strong attention to detail and the ability to multitask in a fast-paced environment
  • Knowledge of current hospice practices, regulations, and standards
  • Commitment to the philosophy of palliative care and ability to serve as an advocate for patients and their families
